Purpose: To compare the accuracy of intraocular lens (IOL) toric power calculation based on posterior corneal surface neglected, estimated or measured in the extreme ametropic population.

Methods: A retrospective monocentric analysis of study data was conducted based on patients from the 1st November 2019 and the 1st January 2023 who had an axial length of <22mm or >26mm. The absolute prediction error (APE) of astigmatism was calculated based on the keratometry (K) and total keratometry (TK) measured 4 weeks after IOL implantation. APE was calculated as the difference between the postoperative refraction and the refractive error predicted by two classic IOL calculation methods (Barrett Toric Calculator and Holladay K) and two new formulas developed for TK (Barrett TK Toric and Holladay TK).

Results: Data are currently collected and will be presented during the meeting.

Conclusions: Given the increasing patients’s expectations regarding glasses independance after cataract surgery, accurate astigmatism measurments is critical
